All tax forms in menu 'Tax forms wizard' use filter by check date

Forms 940:

  • value in box 3 is equal to Total payments (Reports -> Payroll reports -> Employee history -> Employee history summary -> Total payments)
  • value in box 4 is equal to difference between Total payments and FUTA Gross (Reports -> Payroll reports -> Payroll categories -> select FUTA -> Gross)

Form 941 (quarter)

  • Medicare
    • value in 'box 5c column one' is equal to Medicare taxable amount (Reports -> Payroll reports -> Payroll categories -> select category Tax Medicare -> Column Taxable)
    • value in 'box 5c column two' is calculated on the report
    • value in 'box 5d column two' equal to difference between (amount in box 5c column two) and (Medicare paid amount + Medicare ER paid amount). To get paid amounts run report for Tax Medicare and Payroll tax Medicare ER(Reports -> Payroll reports -> Payroll categories) 
    • value in 'box 5d column one' is calculated on the report
